Frequently Asked Questions

Does the Church require a permission slip for every activity?

No — General Handbook 20.5.5 specifies that the Permission and Medical Release Form is required for activities involving overnight stays, travel outside the local area, or higher-than-ordinary risks. Routine weekly activities like Sunday class or a local park activity typically don't require a formal form, though local leaders may choose to use one.

Can one form cover multiple activities?

The official Church form is designed to be completed per event. Each form should list a specific activity with dates. You can, however, keep the same medical and emergency contact information on file and ask parents to update it as needed.

Do adult leaders also need to complete the form?

Yes. General Handbook 20.5.5 specifies that each participant, including leaders, completes this form for qualifying activities.

Is there a digital option so parents don't have to print anything?

Yes — instead of printing and collecting paper, you can share a digital link that parents fill out and sign on their phone. Simple Permission Slip lets you upload the form once, share a link or QR code, and collect all signed submissions in one place. No printing, no chasing parents down the hall.

Skip the Paper Altogether

Paper permission slips get lost, arrive incomplete, and create a last-minute scramble before every activity. If you're tired of counting forms in the parking lot, there's a better way.

Upload your permission slip template once. Share a link or QR code with parents. They sign on their phone in minutes — you see all responses collected in one dashboard. No printing required. Works for every LDS youth activity: Girls Camp, FSY, trek, service projects, and more.
